To help encourage sportsmanship and quality of umpiring within our league, please complete this form to rate teams and umpires.

Game Details


Team Fair Play Rating

Please rate your opponent using the following scale:

5 – Outstanding: The team positively influenced the game through sportsmanship and is a role model for other teams, and demonstrated respect for opponents and officials and a satisfactory knowledge of the sport appropriate for the level of play in order to maintain flow of game. Either no cards were received, or cards received could have reasonably not been given.

4 – Above Average: Overall, the team demonstrated respect for opponents and officials and a satisfactory knowledge of the sport appropriate for the level of play in order to maintain flow of game. Only green cards, if any, were received, or could have reasonably been received, by the team’s players.

3 – Average: Overall, the team demonstrated respect for opponents and officials and satisfactory knowledge of the sport appropriate for the level to play to maintain flow of game although one or both areas has/have room for improvement. Only green cards or no more than one yellow card was received, or could have reasonably been received, by the team’s players.

2 – Below Average: The team did not demonstrate either: sufficient respect for opponents and/or officials overall, or knowledge of the sport appropriate for the level of play in order to maintain flow of game. More than one yellow card was received by the team’s players, or the attitude or conduct of play of the team overall was such that these cards could have reasonably been given.

1 – Needs Improvement: Substantial improvement with respect to sportsmanship towards opponents and/or officials is required and/or knowledge of sport in relation to the level of play is inadequate to the point it compromises safety. More than one yellow or red card was received by the team’s players, or the attitude or conduct of play of the team overall was such that these cards could have reasonably been given.


Umpire Fair Play Rating

Please rate the umpire(s) using the following scale:

5 – Outstanding: The umpire’s game management skills and attitude positively influenced the game. The umpire made fair and consistent calls, and invoked trust through sound decision-making, clear communication, confidence, preparedness, and knowledge of the sport. Good flow and timing of the game was maintained, and the use of advantage was appropriate for the level of play. The umpire had good positioning and worked well with and provided support to the other official(s).

4 – Above Average: The umpire had a positive attitude, made fair and consistent calls, and demonstrated adequate decision-making skills, confidence, preparedness, and knowledge of the sport. The umpire demonstrated good communication and maintained reasonable flow and timing of the game. A good understanding of the advantage rule was demonstrated, with the umpire being able to put the rule into practice during the game. The umpire had adequate positioning and made a concerted effort to work with and support the other official.

3 – Average: The umpire demonstrated a positive attitude. Although more practice may be needed to increase confidence and to hone communication and decision-making skills, the umpire demonstrated potential with a desire to learn and adequate knowledge of the game overall. The umpire was able to maintain control of the game and made an effort to support the other official(s).

2 – Below Average: The umpire did not exhibit a positive attitude and/or a desire to learn or improve. Game management, decision-making, and/or communication skills may have needed improvement. Calls may not have been consistent, and/or the flow and timing of the game could have been better. The umpire also may not have displayed a teamwork mindset in working with the other official(s).

1 – Needs Improvement: The umpire did not exhibit a positive attitude nor a willingness to improve or learn. The umpire did not have sufficient control of the game (i.e. many calls were missed or were inconsistent, and/or inappropriate player conduct was not addressed). Communication was not present, and the game lacked flow. Knowledge of the sport appeared to be lacking, or there was a general disinterest in the game.
